Home Health Certification And Plan Of Care (POC)

Home health care services provided under this chapter must be provided under a POC established and approved by a physician, and the care must be certified by a physician. Please see the Medicare Benefit Policy Manual, Chapter 7, Sections 30.2 and 30.5 for the documentation requirements for the POC and physician certification.
