Listing Of Government Claims Offices

1.0 Uniformed Services

The Uniformed Services and the Department of Health and Human Services (DHHS) have designated the following offices to receive injury claims (diagnoses codes as referenced in Chapter 10, Section 5) and Personal Injury Statements (Department of Defense (DoD) Document (DD) Form 2527) for possible recovery under the Federal Medical Care Recovery Act (FMCRA). The appropriate claims office is selected based upon the sponsor’s branch of service and the state in which the accident occurred. Usually, that is the state in which care was provided; if the DD 2527 does not clearly state the location of the accident, the contractor shall assume that the accident occurred in the state in which medical care was received. Jurisdiction for overseas claims is set forth in paragraph 2.0.
